How an on-line case assessment works
After you submit your payment, sign the client agreement and fill out the behavior history form, we will move into an extensive follow-up question information gathering phase, you will be asked to complete a pain questionnaire and obtain video footage of various things about your pet. This allows me to gain a comprehensive understanding of the situation so that I can make as accurate of an assessment as possible. It is important that you attempt to answer all the questions presented in a timely manner. Following review, you will receive an assessment, risk assessment if applicable, prognosis, and a summary of how I would approach your case.
You will need to be prepared for an EXTENSIVE information collection process. It is crucial that I obtain all the necessary information in order to make an accurate assessment and appropriate treatment plan.
